Sponsors may use this form to authorize CPCC to invoice them for student tuition, fees and books / supplies. We will contact the sponsor to verify the information before processing.

This form MUST BE SIGNED AND SUBMITTED to the Sponsored Program office in order for your loan to be certified. It contains important information regarding your responsibilities when using an alternative loan at CPCC.
