In order to purchase a departmental Smartphone for an employee, the Cellular Phone/Data Access Employee Agreement form must be completed, signed and sent to Cell Phone Administrator within ITS.

Please take a moment to review the Cell Phone Process to familiarize yourself with the purchasing, delivery, and programming procedures.

Once processed, the phone will be ordered and upon receipt, sent to Terrell 107 (ITS Helpdesk) for activation and programming. You will be notified via email once this device has been received and is ready to be picked up.
