Student Accident Policy Summary for 2008/2009 Period
Policy Information
- Cost will be $1.50 per semester.
- Coverage starts at the beginning of the term and stops at the end of the term.
Enrollment:
- Specific classes require student accident insurance and the fee will be included in your tuition.
- If a student is not registered in one of the specific classes and wishes to have this coverage, payment can be made during registration at any Cashier's desk. Coverage begins when tuition and the $1.50 insurance fee are paid.
Accident Claim
The policy offers primary coverage if the student has no other accident or health insurance coverage.
If other accident or health insurance coverage is in force, the policy becomes secondary.
1. Fill out the Student Accident Insurance Claim Form ( PDF )
2. Email or fax the completed claim form to the Director of Administrative Services/Insurance, Harper Campus for signature (pat.emch@cpcc.edu or Fax 704-330-4455.)
3. Form will then be returned to student, and the student must mail the completed and signed form to the insurance company along with any invoices or bills.
